Tolstoy Foundation Rehabilitation & Nursing Center provides a homelike environment for those in need of long-term care and short-term rehabilitation. Throughout its 42 years of service to the elderly, the commitment to excellence has remained steadfast. Resident needs are addressed through care plans, interdisciplinary team meetings and the Resident’s individual preferences. Resident family members and/or significant others, as well as the Resident are considered part of the interdisciplinary team and are encouraged to voice any concerns they have.
The admission process is initiated by evaluating the prospective client, using a Patient Review Instrument (PRI). This review is conducted by a certified assessor, either within the hospital by the Discharge Planner, or by a visiting nurse if the client is coming from the community. Completion and submission of the PRI to TFRNC is the first step toward admission.
After the PRI is reviewed, an application packet must be completed by a family member or friend. This packet asks for information about the prospective Resident's finances, Advanced Directives (healthcare proxy or living will), social information, citizenship and funeral arrangements. Along with the completed application, certain documents are required; they include but are not limited to: a copy of the Social Security and Medicare cards, birth certificate, naturalization papers or green card, any supplemental insurance cards, and all bank statements for the most recently past 36 months.
The private bed rate is $310.00 a day, plus additional charges for pharmaceutical items.
